It does not replace the privacy notices of USAC, the National Verifier, state Lifeline programs, or other services you may use during eligibility verification.
Information the site may collect
Contact information
If you use an application or contact interface, the website may ask for information such as your name, phone number, optional email address, preferred contact method, topic and message.
Preliminary application information
The application asks for state, ZIP code, a preliminary eligibility route, qualifying-program selection or approximate household size and income, and basic contact details. A secure server route can deliver a validated application event to the provider’s configured webhook destination. The form is not the National Verifier and does not make an official eligibility decision.
Sensitive eligibility information
The website does not request Social Security numbers, EBT numbers, Medicaid numbers, identity-document uploads or benefit-document uploads through its general forms. Do not place sensitive eligibility records in the Contact form, application form or general email.
Technical and log information

Choices and rights that apply to future stored application data should be documented when that backend is introduced.

This website is intended for consumers seeking Lifeline-related connectivity information and provider service. It is not designed to knowingly collect personal information directly from children through its forms. Parents or guardians should handle provider inquiries for minors when appropriate.
Government and program websites
Links to FCC, USAC, LifelineSupport.org or other official resources lead to websites with their own privacy practices. Government Tablet does not control those external policies.
